DISCLAIMER AND CANCELLATION POLICY

22 Wellness (Heather Kolisnyk) employs ‘active imaginal’ techniques similar to hypnosis to facilitate the client’s quest for self- improvement. Specific techniques may include: Spiritual Philosophy, Body Movement or psychodrama involving the body, Guided Meditation/Imagery, Shamanic Journeying, Childhood or Age Regression, Past-Life Regression, ‘Hands-on’ Healing, Reiki, Energy Healing, or Psychic Insight. I (the client or participant) affirm that these services are appropriate for me and do not conflict with any existing or past medical, physical or psychiatric condition or treatment. Heather Kolisnyk is not a mental health counselor, trained psychologist or a medical doctor. I understand that the services offered are an alternative and/or complementary service.  

I hereby waive and release 22 Wellness (Heather Kolisnyk) from any and all liability of any nature as a result of my session or any services received.  I also agree to defend, indemnify and to hold harmless 22 Wellness (Heather Kolisnyk) and all parties involved in the services from all claims, or claims by any member of my family or person acting on my behalf. I shall personally assume all responsibilities and liabilities for any loss or injury which may be alleged to have been caused directly or indirectly to any person or thing while participating in this session or receiving these services or while on the premises where this session occurred.

NOTE:

If you are in need of professional care from a doctor, counsellor, psychiatrist, lawyer, or financial advisor please obtain these services.  Always follow the advice of your doctor or other professional medical practitioner. 22 Wellness (Heather Kolisnyk) reserves the right to refuse service to anyone.  Services will not be provided to anyone under 18 years of age without written consent from a parent or legal guardian.  No warranty or guarantee is given, expressed or implied, for satisfactory results from a session or other services provided.
